[CHANGES]
Beginning Tuesday, May 20, 2014, the Florida Medicaid Sanctions database will return fewer names as we limit its entries to providers who have been sanctioned by Florida’s Medicaid office.
To assist you in ruling out potential Florida Medicaid matches, we will also be adding the following new fields:
- Medicaid Provider Number
- License Number
- NPI Number
- Provider Type
- Sanction Date
- First and Last Name
- Email Address
- Office Phone Number
- Company Name
- 3-digit Customer ID (CusID appears in the search history listing) if available
